Description and Service Details

Description (Service) Outpatient, community-based program for individuals 14-35 years old experiencing early stages of psychosis * clinical partnership between Halton Healthcare, Joseph Brant Hospital, North Halton Mental Health Clinic, Halton ADAPT and the Schizophrenia Society of Ontario * this recovery focused program helps individuals achieve their goals at work, at school and within their social networks, thereby reducing the impact psychosis can have on one’s life * team includes psychiatrists, nursing, social work, occupational therapists, peer support and addiction counsellors


Services include:
  • psychiatric assessment and treatment
  • medication and symptom management
  • case management, group and individual counselling
  • family education and support
  • relapse prevention planning and strategies
  • addictions counselling
  • peer support
  • community program referral and illness management instruction after discharge
  • assistance with education, work, and housing

Eligibility People between the ages 14 to 35 years old who are experiencing symptoms of psychosis and who have received either no previous treatment or 6 months or less of treatment for psychosis and who live in the Region of Halton
