Description and Service Details

Description (Service) Provides long-term case management to enable individuals with severe and persistent mental illnesses to attain self-sufficiency, stability and integration into the community * team of nurses and social workers provides long term slow paced rehabilitation * services are ongoing as long as client needs support * discharge occurs when the client moves outside Halton, refuses or no longer requires service or demonstrates ongoing violent or aggressive behaviour * partner of the Halton Homes Program, working in collaboration with Summit Housing & Outreach, Support & Housing Halton and Joseph Brant Hospital

Services include:
  • psychiatric assessment and treatment
  • medication and symptom management
  • case management, group and individual counselling
  • education about symptoms and treatments
  • relapse prevention planning and strategies
  • collaboration and referral to community partners
  • family/caretaker education and support
