Description (Service)

The goal of the RCM is to ensure vulnerable populations receive appropriate clinical support and monitoring in the community, including escalation to medical practitioners or acute care, where necessary * helps to reduce unplanned visits to the Emergency Department and/or unplanned re-admissions and develop sustainable self-management techniques for the identified patient populations supported through education and resources * allows patients with COVID, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), asthma, interstitial lung disease (ILD), pneumonia, and other respiratory illnesses, to manage their symptoms at home with technology and nursing support provided to them


The program is a partnership between Halton Healthcare (HH), Connected Care Halton Ontario Health Team (CCHOHT) and Ontario Health atHome * Aetonix provides the remote care management technology platform * paramedics visit patients at home and provide them with a tablet, vital signs collection kit and assist with set-up and a OHaH nurse will follow-up once referral is received
