Description (Service) | Provides comprehensive assessment, rehabilitation/treatment and supportive counselling to clients with complex and/or serious and persistent mental health disorders, living in North Halton * interprofessional staff include psychiatrists, nurse practitioners, dietitians, social workers, community support workers, child and youth counsellors, occupational therapists, teachers, peer support, etc * referrals are made by doctors through One-Link * services are also offered to older persons with mental health issues, including behavioural and psychological complications associated with neurodegenerative diseases Services provided include:

Application | Individuals seeking services are asked to first discuss their concerns with their family doctor and if a referral is required to have their doctor make the referral on their behalf to One-Link where there is no family doctor, staff will assist someone in locating a doctor to work with all referrals require the expressed consent of the person being referred for service |
